Begin with unresolved work
Before the outgoing responder leaves, list conversations that still require action from the team. Separate them from items waiting on the customer or an external specialist. A busy transcript is not necessarily unfinished work, and a short exchange can contain an important promise. Focus the handoff on what must happen next. Keep the supported conversation record as the reference rather than creating a second unofficial history in a personal document.
Use a five-field handoff note
For each item, state the question, verified facts, last promise, next action and receiving owner. Include a time expectation only when it has actually been agreed. Label uncertain information clearly and leave out irrelevant personal details. This compact structure helps the next responder act without reconstructing every message. It also makes a missing decision visible: an item with no owner or no next action has not really been handed over.
Require acceptance at the boundary
The incoming responder should confirm they can access the record and understand the next step. If they lack the required authority or knowledge, identify another route before the outgoing person disappears. In a fictional practice exercise, ask the recipient to explain what they would do next and what they would tell the customer. That reveals gaps more reliably than simply checking whether the transfer button returned a success message.
Close the loop the next day
Review whether handed-over promises were kept, changed transparently or lost. Use failures to improve the structure rather than lengthening every note. If the same specialist question repeatedly crosses shifts unresolved, fix the escalation arrangement. Maintain a reduced-coverage rule for days when the incoming person is absent. A handoff process should work under ordinary interruptions, not depend on everyone being available for an informal conversation at exactly the right moment.
